At Cradley Surgery every other Tuesday.The role of a community mental health nurse is to work within different parts of the community to provide people with specialist mental healthcare and treatment. They work alongside other mental health specialists, building strong working relationships with patients.

These are staff members who do not work on the Cradley Surgery site, they work remotely and will contact patients via telephone. The telephone number which they contact you on may show up as a withheld.

Frailty Team
The role of our occupational therapists and frailty team is to recognise people who may becoming frailer and help them to manage this as far as possible. As well as helping those people and their families/ carers to plan for the future. Sian and Heulwen work for the East PCN and also work alongside Ledbury, Colwall and Nunwell surgeries. Sian and Heulwen are contactable via the Surgery and are often out and about visiting patients or popping into the Surgery to liase with the team.
