How to Register
We can only register patients who live within our practice area.
We need you to complete the appropriate forms which are available from reception (One of these forms can be completed in advance by printing off our New Patient Questionnaire). If you have a medical card please bring this with you but we do have alternative forms should you have mislaid your existing card.
Ths practice area is centred around Cradley but includes outlying villages and certain parts of Malvern as shown by the map.

8.00am - 6.00pm
Surgery Hours
Monday - Friday
Mornings - 8.30am - 11.30am
Evenings - 4.00pm - 5.30pm
Emergency Surgery - 10.30am - 1.00pm
