How to Register

We are happy to accept new patients at the Practice, however we can only register patients who live within our practice area.

We need you to complete the appropriate forms which are available from reception or to print from here. www.nhs.uk/nhsengland/aboutnhsservices/documents/doctors/gms1.pdf    and  our New Patient Questionnaire. If you have a medical card please bring this with you but we do have alternative forms should you have mislaid your existing card. Our Practice leaflet, containing surgery times and general information, is also available at reception on request.

Ths practice area is centred around Cradley but includes outlying villages and certain parts of Malvern as shown by the map.

Monday - Friday
8.00am - 6.00pm

Surgery Hours

Monday - Friday
Mornings - 8.30am - 11.30am
Evenings - 4.00pm - 5.30pm

Emergency Surgery - 10.30am - 1.00pm
