Going abroad?

For a Travel risk assessment form print out and fill in our Travel Form. Please phone the surgery 14 working days after submitting the completed form for the result and for an appointment if necessary.

ARRS (additional roles reimbursement scheme)

  • Physio, Graham is onsite for appointments only. Graham only comes to Cradley on Thursdays however we can also offer remote calls.
  • Social Prescriber Amy, is onsite on Thursdays for appointments only
  • Health and wellbeing coach Mark is onsite on Tuesdays (starting September 10th) for appointments only
  • Pharmacist, Steffan is onsite for appointments only on Tuesdays, remote pharmacists are available on other days
  • Remote Advanced nurse practioners, Rebecca is available on Thursdays and Fridays for telephone calls
  • HRH (Herefordshire remote healthcare)

Medical reports

The doctors undertake some work which is not provided for within the NHS, for instance medical reports that relate to employment or insurance.  We use BMA guidance to set the charges we make and require payment to be made to the surgery prior to carrying out any work.

Certain partners within the practice have a special interest in Diabetes, Asthma and Heart Disease Prevention.

Maternity and child health surveillance

The surgery provides maternity & child health surveillance services. We also provide family planning advice and emergency contraception to all patients in the strictest confidence. Please see our receptionists for an appointment.

Medication Delivery Service

We have free medication delivery service available. Full information how to apply for the delivery service and how to arrange for your repeat medication to be ordered are available from the surgery.

We do not take requests for repeat medication over the telephone. These should be made by posting, calling in to the surgery, our online consultation tool or using the NHS App.  For the Thursday delivery we must have received your request by 08.00 on the preceding Tuesday

You will need to complete a Delivery Service Agreement form which can be downloaded by clicking here. The service operates on a Thursday and you will need to be at home to receive your medication unless you have made alternative arrangements. Please contact the surgery for more details.

You will still need to see the doctor at least every six months for them to confirm that you can continue to take the medication prescribed and at least annually you will need to have a Medication review with the doctor.

We do not take requests for repeat medication over the telephone. These should be made by posting, calling in to the surgery, faxing or using your unique PIN (for details please contact the surgery). For the Thursday delivery we must have received your request by 08.00 on the preceding Tuesday.

We do reserve the right to refuse requests where we feel it would put members of the practice team in jeopardy to deliver to you.

Repeat Prescriptions

Regular medication may be supplied , if agreed by your doctor.

Please request your repeat prescription by returning the right hand side of the slip to the surgery, having ticked which items you require. You can also use the NHS app or our online consultation tool to request repeat prescriptions.

We do not accept prescription requests over the telephone. 

We would ask that you give us 3 working days notice before you need your medicines as these have to be ordered from our suppliers.

Dispensing

We are a dispensing practice.

This means that if you live more than a mile from a community pharmacy or chemist we are able to dispense your medication at the surgery. Occasionally due to difficulties getting certain medications from our suppliers you may be given a prescription to take to the chemist.
