- Worcestershire
- For Malvern or Worcester patients can self-refer, the Surgery can send you a link to do so or alternatively please visit; www.hacw.nhs.uk/movetoimprove.
- Herefordshire
- If patients wish to go Herefordshire, e.g Ledbury or Bromyard, the process is slightly different. Patients cannot self-refer and would need to speak with, or see the GP, for onward referral. Please complete our online consultation form via our website or call us 01886 880 207.
